OBJETIVE: To assess the ability of an artificial intelligence software to detect pneumothorax in chest radiographs done after percutaneous transthoracic biopsy. MATERIAL AND METHODS: We included retrospectively in our study adult patients who underwent CT-guided percutaneous transthoracic biopsies from lung, pleural or mediastinal lesions from June 2019 to June 2020, and who had a follow-up chest radiograph after the procedure. These chest radiographs were read to search the presence of pneumothorax independently by an expert thoracic radiologist and a radiodiagnosis resident, whose unified lecture was defined as the gold standard, and the result of each radiograph after interpretation by the artificial intelligence software was documented for posterior comparison with the gold standard. RESULTS: A total of 284 chest radiographs were included in the study and the incidence of pneumothorax was 14.4%. There were no discrepancies between the two readers' interpretation of any of the postbiopsy chest radiographs. The artificial intelligence software was able to detect 41/41 of the present pneumothorax, implying a sensitivity of 100% and a negative predictive value of 100%, with a specificity of 79.4% and a positive predictive value of 45%. The accuracy was 82.4%, indicating that there is a high probability that an individual will be adequately classified by the software. It has also been documented that the presence of Port-a-cath is the cause of 8 of the 50 of false positives by the software. CONCLUSIONS: The software has detected 100% of cases of pneumothorax in the postbiopsy chest radiographs. A potential use of this software could be as a prioritisation tool, allowing radiologists not to read immediately (or even not to read) chest radiographs classified as non-pathological by the software, with the confidence that there are no pathological cases.

Our aim was to describe the research practices of doctoral students facing a dilemma to research integrity and to assess the impact of inappropriate research environments, i.e. exposure to (a) a post-doctoral researcher who committed a Detrimental Research Practice (DRP) in a similar situation and (b) a supervisor who did not oppose the DRP. We conducted two 2-arm, parallel-group randomized controlled trials. We created 10 vignettes describing a realistic dilemma with two alternative courses of action (good practice versus DRP). 630 PhD students were randomized through an online system to a vignette (a) with (n = 151) or without (n = 164) exposure to a post-doctoral researcher; (b) with (n = 155) or without (n = 160) exposure to a supervisor. The primary outcome was a score from - 5 to + 5, where positive scores indicated the choice of DRP and negative scores indicated good practice. Overall, 37% of unexposed participants chose to commit DRP with important variation across vignettes (minimum 10%; maximum 66%). The mean difference [95%CI] was 0.17 [- 0.65 to 0.99;], p = 0.65 when exposed to the post-doctoral researcher, and 0.79 [- 0.38; 1.94], p = 0.16, when exposed to the supervisor. In conclusion, we did not find evidence of an impact of postdoctoral researchers and supervisors on student research practices.Trial registration: NCT04263805, NCT04263506 (registration date 11 February 2020).

Introducción: Se cumple ahora más de una década del inicio de la hipotermia terapéutica (HT)en Espa ̃na, la única intervención neuroprotectora que ha venido a ser práctica estándar en eltratamiento de la encefalopatía hipóxico-isquémica perinatal (EHI). El objetivo de este artículoes ofrecer un panorama actual y presentar las controversias surgidas alrededor de la aplicaciónde esta terapia. Desarrollo: En esta década se ha implantado con éxito la HT en la gran mayoría de los hospitalesterciarios de Espa ̃na y más del 85% de los recién nacidos con EHI moderada-grave reciben estaterapia. Entre los aspectos que pueden mejorar la eficacia de la HT están su inicio precoz dentrode las primeras 6 h de vida y el control de factores comórbidos asociados a la asfixia perinatal. En los pacientes con EHI moderada el inicio después de las 6 h parece mantener cierta eficacianeuroprotectora. Una duración de la HT mayor de 72 horas o un enfriamiento más profundo noofrecen mayor eficacia neuroprotectora y aumentan el riesgo de efectos adversos. Aspectosno bien aclarados aún son la sedación durante la HT y la aplicación de esta intervención a losneonatos con EHI leve y en otros escenarios. La información pronóstica y su marco temporal esuno de los aspectos más desafiantes. Conclusiones: La HT es universal en países con recursos económicos, aunque existen puntos de controversia no resueltos. Si bien es un tratamiento generalizado en nuestro país, falta disponerde dispositivos para el traslado de estos pacientes y su centralización.(AU)

Ion stopping in warm dense matter is a process of fundamental importance for the understanding of the properties of dense plasmas, the realization and the interpretation of experiments involving ion-beam-heated warm dense matter samples, and for inertial confinement fusion research. The theoretical description of the ion stopping power in warm dense matter is difficult notably due to electron coupling and degeneracy, and measurements are still largely missing. In particular, the low-velocity stopping range, that features the largest modelling uncertainties, remains virtually unexplored. Here, we report proton energy-loss measurements in warm dense plasma at unprecedented low projectile velocities. Our energy-loss data, combined with a precise target characterization based on plasma-emission measurements using two independent spectroscopy diagnostics, demonstrate a significant deviation of the stopping power from classical models in this regime. In particular, we show that our results are in closest agreement with recent first-principles simulations based on time-dependent density functional theory.

Beyond fifth generation (5G) communication systems aim towards data rates in the tera bits per second range, with improved and flexible coverage options, introducing many new technological challenges in the fields of network architecture, signal pro- cessing, and radio frequency front-ends. One option is to move towards cell-free, or distributed massive Multiple-Input Multiple-Output (MIMO) network architectures and highly integrated front-end solutions. This paper presents an outlook on be- yond 5G distributed massive MIMO communication systems, the signal processing, characterisation and simulation challenges, and an overview of the state of the art in millimetre wave antennas and electronics.

INTRODUCTION: The study aims to quantify the types of antiepileptic drugs (AED) prescribed in neurology consultations. MATERIAL AND METHOD: This descriptive, observational study included a sample of 559 patients older than 14 years, diagnosed with epilepsy, and receiving pharmacological treatment. Data were collected at outpatient consultations by 47 Spanish neurologists in May 2016. Epilepsy was defined based on the International League Against Epilepsy classification. According to the year of marketing, AEDs were categorised as classic (before 1990) or new (after 1990). We performed a descriptive analysis of qualitative and quantitative variables. RESULTS: Female patients accounted for 54.6% of the sample. Mean age was 42.7 years; mean age of onset was 22.4. Regarding epilepsy type, 75.7% of patients experienced partial seizures, 51.5% were symptomatic,32.4% had refractory epilepsy, 35.6% had been seizure-free for the previous year, and 59.2% had associated comorbidities.A total of 1103 AED prescriptions were made; 64.6% of prescriptions were for new AEDs; 85.4% of patients received new AEDs. Patients received a mean of 2 AEDs (range, 1-5). A total of 59.6% of patients received polytherapy.The most frequently prescribed AEDs were levetiracetam (42.6%), valproic acid (25.4%), lamotrigine (19.5%), carbamazepine (17.9%), and lacosamide (17.5%). No AED was employed exclusively as monotherapy. The most frequently prescribed AEDs for generalised and partial seizures were valproic acid (48.2%) and levetiracetam (43.2%), respectively. Valproic acid was less frequently prescribed to female patients. Patients with refractory epilepsy or with associated comorbidities were more frequently prescribed a combination of new and classic AEDs (48.7% and 45.6%, respectively) than only one type of AED. CONCLUSIONS: The majority of patients received new AEDs. The combination of classic and new AEDs was more frequently prescribed to patients with refractory epilepsy or with associated comorbidities.

The aim of this work was to elucidate the role of the secondary metabolites produced by B. amyloliquefaciens BUZ-14 against B. cinerea, M. fructicola, M. laxa, P. digitatum, P. italicum and P. expansum both in vitro and in planta. The entire cell free supernatant (CFS) and the lipopeptide fraction (LPF) showed similar antifungal activities, completely inhibiting all the fungi at dilutions of 1:24 or even lower, whereas the non-butanolic fraction (NBF) barely inhibited the fungi. However, when the LPF and CFS were applied on fruit, only brown rot in peaches and blue rot in apples was totally inhibited. The main families of metabolites in the LPF were iturin A, fengycin and surfactin with maximum concentrations of 407, 853 and 658 µg mL-1, respectively. Subsequently, a TLC-bioautography revealed iturin A as the key metabolite in the inhibitions and allowed us to establish in vivo MICs of 16.9 and 33.9 µg mL-1 for Monilinia species and P. expansum, respectively. The application of 24 h-old BUZ-14 cultures suppressed brown rot in peaches and also blue rot in apples but failed to inhibit the other diseases. However, BUZ-14 was only able to grow and produce iturin A in peaches so we can deduce that the amount of iturin A brought with the cultures (36 ±â€¯14 µg mL-1) could be enough to control both diseases. The strong antifungal activity of the iturin A present in the BUZ-14 CFS suggests that it could be successfully used for postharvest disease control. However, future research is necessary to maximize the iturin A production by B. amyloliquefaciens BUZ-14 in order to optimize a commercial application.

A obtenção de cariótipos de peixes desempenha um papel importante em estudos de citotaxonomia e evolução cromossômica das espécies. No entanto, poucos sistemas semi ou completamente automatizados para a obtenção de cariótipo de peixes estão disponíveis. Este trabalho propõe e avalia uma ferramenta baseada em imagens que auxilie a montagem de cariótipos de peixes. As espécies analisadas foram Hopliasmalabaricus (Bloch, 1794); Hypostomusancistroides (Ihering, 1911) e Parauchenipterusgaleatus (Linnaeus, 1766); popularmente conhecidas como traíra, cascudo e bagre-sapo, respectivamente.Um total de 100 metáfases foi analisado por dois métodos: 1 - geração semiautomática de cariótipo e 2 - geração automática de cariótipo. A avaliação do sistema foi feita por meio da correlação de Pearson, gráficos de diferenças e tabelas de contagens,utilizando-se como referência a média das contagens feitas por quatro usuários. No método 1, quatro usuários realizaram contagens e apresentaram correlação interobservador de r≥ 0.997. O número total de cromossomos identificados pelo método 1 foi 4348 e, para o método 2, foi 4135,o que resultou em uma identificação automática de aproximadamente 95,1% dos cromossomos, resultando em correlação entre os dois métodos de r= 0.93. Conclui-se que a ferramenta pode ser inserida no procedimento de cariotipagem de peixes para acelerar o processo com níveis aceitáveis de exatidão.(AU)


Fish karyotyping plays an important role in studies of cytotaxonomy and chromosomic evolution of species. However, few semi or completely automated fish karyotyping systems are available. This work proposes and evaluates an image-based tool to assist fish karyotyping. The analyzed species were Hopliasmalabaricus (Bloch, 1794), Hypostomusancistroides (Ihering, 1911), and Parauchenipterusgaleatus (Linnaeus, 1766). In Portuguese, these species are commonly referred to as traíra, cascudo, and bagresapo, respectively. A total of 100 metaphases were analyzed through two methods: (1) semi-automatic karyotype generation and (2) automatic karyotype generation. The results were analyzed using Pearson correlation, difference graphs and counting tables. The reference used for the evaluation of the system was the average of the counts made by four experts. In method 1, four users performed counts with interobserver correlation of r≥ 0.997. The total number of chromosomes identified by method 1 was 4348 and method 2 was 4135, excluding false positives, resulting in an automatic identification of approximately 95,1% of the chromosomes, resulting in a correlation between the methods of r= 0.93. The results indicate that the tool can be introduced for fish karyotyping procedures contributing for accelerating the process with acceptable accuracy.(AU)

Introduction: Chondrocalcinosis is a common joint condition in people over 50, characterized by the deposition of calcium pyrophosphate crystals that causes calcification in cartilage structures. Although its prevalence in the general population is 5%, sclerochoroidal calcifications are a rare finding in this pathology. Objective: Describe the case of a 60-year-old man with choroidal calcification in the left eye, associated with radiological findings of chondrocalcinosis, whose metabolic report showed parameters compatible with hyperparathyroidism. Clinical case: Male patient from Mexico City, smoker for 45 years, with medical history of calcium pyrophosphate deposition disease at age 16, attended routine eye examination. A left eye funduscopy revealed an irregularly shaped white-yellowish subretinal lesion with well-defined edges inside the upper temporal arch. Ophthalmological paraclinical studies were performed, and a choroidal hyperreflectic paramacular plate image, which did not involve the retina layers, was corroborated with five-line optical coherence tomography. Conclusions: Although the lesions are asymptomatic and do not require treatment, it is of great importance to make a differential diagnosis with choroidal metastases, choroidal melanoma, choroidal osteoma and lymphoma, since these pathologies do require more aggressive treatment.


Introducción: la condrocalcinosis es una afección articular común en personas mayores de 50 años, caracterizada por el depósito de cristales de pirofosfato de calcio que produce calcificación en estructuras cartilaginosas. Aunque su prevalencia en población general es del 5%, las calcificaciones esclerocoroideas son un hallazgo poco frecuente en esta patología. Objetivo: describir el caso de un hombre de 60 años de edad con calcificación coroidea en el ojo izquierdo, asociada a hallazgos radiológicos de condrocalcinosis, cuyo reporte metabólico evidenció parámetros compatibles con hiperparatiroidismo. Caso clínico: paciente de la Ciudad de México con antecedentes de enfermedad por depósito de pirofosfato de calcio hace 16 años y tabaquismo desde hace 45 años, acudió a revisión oftalmológica de rutina. A la fundoscopia de ojo izquierdo se observó por dentro de arcada temporal superior, una lesión subretiniana blanco-amarillenta de forma irregular con bordes bien definidos. Se le realizaron estudios paraclínicos oftalmológicos y se corrobora con tomografía de coherencia óptica de cinco líneas, imagen en placa paramacular hiperrefléctica coroidea, que no involucra las capas de la retina. Conclusiones: aunque las lesiones son asintomáticas y no requieren tratamiento es de gran importancia hacer un diagnóstico diferencial con metástasis coroideas, melanoma coroideo, osteoma coroideo y linfoma, ya que estas patologías sí implican un tratamiento más agresivo.

An improved gas sample introduction interface is developed and characterized for gas chromatography coupling and for direct injection of volatile organic compounds (VOCs), in a pulsed glow discharge (pulsed-GD) ion source coupled to a time of flight mass spectrometer (TOFMS) that is typically used for direct solid analysis. The novel interface allows the introduction of the analytes in the flowing afterglow region of the GD (a few mm away from the negative glow region) to reduce plasma quenching effects. Analyte ion signals are acquired in the temporal afterglow region, where low fragmentation of the molecular species is produced, providing useful qualitative and quantitative molecular information (e.g. molecular ion). Analytical capabilities of the pulsed-GD ion source with the novel gas sampling interface provides improved performance compared to previous designs. In particular, limits of detection for the analysis of VOCs in air were below (better) that legally established limits according to Directive 2008/50/EC of the European Parliament.

The present study focuses on soil washing enhancement via soil pretreatment with nanoscale zero-valent iron (nZVI) for the remediation of potentially toxic elements. To this end, soil polluted with As, Cu, Hg, Pb and Sb was partitioned into various grain sizes (500-2000, 125-500 and <125 µm). The fractions were pretreated with nZVI and subsequently subjected, according to grain size, to Wet-High Intensity Magnetic Separation (WHIMS) or hydrocycloning. The results were compared with those obtained in the absence of nanoparticles. An exhaustive characterization of the magnetic signal of the nanoparticles was done. This provided valuable information regarding potentially toxic elements (PTEs) fate, and allowed a metallurgical accounting correction considering the dilution effects caused by nanoparticle addition. As a result, remarkable recovery yields were obtained for Cu, Pb and Sb, which concentrated with the nZVI in the magnetically separated fraction (WHIMS tests) and underflow (hydrocyclone tests). In contrast, Hg, concentrated in the non-magnetic fraction and overflow respectively, while the behavior of As was unaltered by the nZVI pretreatment. All things considered, the addition of nZVI enhanced the efficiency of soil washing, particularly for larger fractions (125-2000 µm). The proposed methodology lays the foundations for nanoparticle utilization in soil washing operations.

Motivation: Molecular profiling techniques have evolved to single-cell assays, where dense molecular profiles are screened simultaneously for each cell in a population. High-throughput single-cell experiments from a heterogeneous population of cells can be experimentally and computationally sorted as a sequence of samples pseudo-temporally ordered samples. The analysis of these datasets, comprising a large number of samples, has the potential to uncover the dynamics of the underlying regulatory programmes. Results: We present a novel approach for modelling and inferring gene regulatory networks from high-throughput time series and pseudo-temporally sorted single-cell data. Our method is based on a first-order autoregressive moving-average model and it infers the gene regulatory network within a variational Bayesian framework. We validate our method with synthetic data and we apply it to single cell qPCR and RNA-Seq data for mouse embryonic cells and hematopoietic cells in zebra fish. The biocontrol potential of the Bacillus amyloliquefaciens strain BUZ-14 was tested against the main postharvest diseases of orange, apple, grape and stone fruit. After characterizing the temperature and pH growth curves of strain BUZ-14, its in vitro antifungal activity was determined against Botrytis cinerea, Monilinia fructicola, M. laxa, Penicillium digitatum, P. expansum and P. italicum. Subsequently, in vivo activity was tested against these pathogens by treating fruit with cells, endospores and cell-free supernatants. The in vitro results showed that BUZ-14 inhibited the growth of all the pathogens tested corresponding to the least susceptible species, P. italicum, and the most susceptible, M. laxa. In vivo tests corroborated these results as most of the treatments decreased the incidence of brown rot in stone fruit from 100% to 0%, establishing 107 CFU mL-1 as the minimum inhibitory concentration. For the Penicillium species a preventive treatment inhibited P. digitatum and P. italicum growth in oranges and reduced P. expansum incidence in apples from 100% to 20%. Finally, it has been demonstrated that BUZ-14 was able to survive and to control brown rot in peaches stored at cool temperatures, making it a very suitable biocontrol agent for application during the post-harvest storage and marketing of horticultural products.

Background: The aim of this paper was to determine the most common etiology of ocular evisceration in a tertiary-level hospital. Methods: Observational and retrospective study in the Ophthalmology Department from January 2013 to December 2016 at the Centro Médico Nacional Siglo XXI. Results: Of the 54 eviscerations, exogenous endophthalmitis was found 25.90%, ocular trauma 22.2%, painful blind eye 14.8%, perforated corneal ulcer and endogenous endophthalmitis both in 13% and perforated infectious corneal ulcers 11.1%; 70.4% were associated with systemic comorbidities and 79.6% were associated with ophthalmologic comorbidities. Conclusions: Exogenous endophthalmitis is the most frequent evisceration; the loss of an eyeball, has important social and labor impact, and economic for the Instituto Mexicano del Seguro Social.


Introducción: El objetivo de este trabajo fue describir la etiología más frecuente de evisceración ocular en un hospital de tercer nivel de atención. Métodos: Estudio observacional y retrospectivo en el servicio de Oftalmología del Hospital de Especialidades del CMN SXXI de enero del 2013 a diciembre del 2016. Resultados: De las 54 evisceraciones, se encontraron como causas principales: la endoftalmitis exógena (25.90%), el traumatismo ocular (22.2%), el ojo ciego doloroso (14.8%), la úlcera corneal perforada y la endoftalmitis endógena, ambas con 13%, así como las úlceras corneales perforadas infecciosas (11.1%); el 70.4% de estas causas estuvieron asociadas a comorbilidades sistémicas y el 79.6% a comorbilidades oftalmológicas. Conclusiones: Las endoftalmitis exógenas son las causas más frecuentes de evisceración; la pérdida de un globo ocular tiene importante repercusión social, laboral, y económico para el Instituto Mexicano del Seguro Social.

Las miocardiopatías son condiciones que afectan al miocardio y generan alteración en la función cardiaca. Dentro de las miocardiopatías, el subgrupo de las restrictivas tiene como principal hallazgo la disminución en el llenado ventricular. A continuación se expone una revisión acerca de las miocardiopatías restrictivas, se analizan sus principales causas, y los hallazgos por resonancia magnética cardiaca y por tomografía computarizada. También se incluyen signos por imagen que ayudan a diferenciar las miocardiopatías restrictivas de la pericarditis constrictiva.


Cardiomyopathies are conditions that affect the myocardium and cause alteration in the cardiac function. Within the cardiomyopathies, the restrictive subgroup has as a main finding the decrease in the ventricular filling. In this manuscript we will review the restrictive cardiomyopathies and discuss their main causes, as well as their imaging findings on cardiac magnetic resonance and computed tomography. We will also include imaging signs that helps to differentiate restrictive cardiomyopathies from constrictive pericarditis.

La disminución de la densidad mineral ósea (DMO), es decir, del volumen de tejido óseo por unidad de volumen del esqueleto, es característica de la osteoporosis, mientras que se ha sugerido que la artrosis se acompaña de un aumento de la DMO a nivel local y sistémico. Para comprobar esta hipótesis analizamos mediante microTAC el hueso trabecular de la cabeza femoral de 10 pacientes con fractura de cadera y 9 con coxartrosis. El análisis no reveló diferencias significativas entre ambos grupos en el volumen de tejido óseo trabecular, ni en los demás parámetros estructurales analizados. Tampoco se encontró una caída significativa del volumen de hueso trabecular con la edad. Esto indica que el hueso de esta región tiene una evolución peculiar. Los mecanismos responsables de ese comportamiento son desconocidos, pero su esclarecimiento podría, quizás, abrir la puerta a nuevos abordajes en el tratamiento de la pérdida de hueso asociada al envejecimiento (AU)


Whereas bone mineral density (BMD) is characteristically low in osteoporosis, it has been postulated that in osteoarthritis BMD is increased. We aimed to check this concept by analyzing bone volumen and structure in the femoral heads of patients with hip fractures (n=10) and with hip osteoarthritis (n=9). Unexpectedly, the analysis of microstructural parameters by microCT did not reveal significant differences between both groups. In addition, we did not find a significant decline in the trabecular bone volume across the age range studied. These results suggest that the evolution of the trabecular bone of the femoral head is different from the age-related decrease of bone mass in other regions of the skeleton. Elucidating the mechanism involved could suggest new approaches to treat the bone loss associated with aging (AU)
